Chicago West’s birth certificate revealed, surrogate not listed in the document (Photo)

The birth certificate of Kim Kardashian and Kanye West's third child Chicago West, has been revealed, but the couple didn't  list the name of their surrogate on their baby’s birth certificate.

According to the official document, the certificate revealed that Chicago West was born at 12:47 AM on Monday, January 15 at Cedars Sinai Hospital in Los Angeles, CA.

The delivery doctor is listed as Dr. Paul Crane, who also delivered North and Saint.
